The Social Work I- Outreach position is a Master's level social work position that provides social work services to enhance at-risk youth’s adjustment and engagement in HIV-related healthcare. Social work services include psychosocial assessment, evidenced-based counseling, case management, and on-call crisis coverage. Outreach services include community-based interventions and home visits to re-engage patients who are at risk of being lost to follow-up.
The ideal candidate has excellent written and oral communication skills, as well as exceptional critical listening and thinking skills. The outreach social worker must collaborate well with other team members and community service providers. Candidates must be knowledgeable about trauma-informed care and comfortable working with a diverse patient population. Experience working with LGBTQ youth and/or patients infected with HIV is preferred
Work Schedule:
Monday- Friday, 8am-5pm with rotating on-call (evenings/weekends) 3-4 times/year.
