As a Social Worker II on this team, you will provide supportive counseling, case management and crisis intervention to the Leukemia patient population. You focus on helping patients and families adjust to the impact of illnesses and any associated lifestyle changes. In this role, you will collaborate with the multidisciplinary team in psychosocial and medical rounds for high acuity cases. Social work services include performing psychosocial assessments, providing counseling and information/referral/case management, sharing on-call crisis coverage and facilitating patient and family groups as well as bereavement follow-up.
