The Transition School Liaison serves as a liaison with school system personnel in order to advocate for the educational needs of patients. Confers with students, parents, and school counselors to resolve academic problems. Collaborates with St. Jude personnel including clinical staff and staff from Psychology, Rehabilitation Services and Social Work to understand each patient's needs and recommendations for the classroom setting. Participates in staff meeting, professional meetings, educational conferences, and teacher training workshops. Attends Individual Educational Plan and 504 Plan meetings for patients in person or through conference calls.
For Teacher a Master's Degree in Education or Special Education from a regionally accredited institution is required;
or for School Psychologist Master's Degree in Education Specialist degree in School Psychology is required;
or for School Social Worker a Master's Degree from an accredited social work program and licensure as a School Services Personnel through Tennessee Department of Education or certification as a Certified School Social Work Specialist through the National Association of Social Workers
Three (3) years of instruction experience, experience as a school psychologist, school social worker or experience as a professional educational advocate
Experience working with/writing IEPs and 504 Plans is preferred. Experience advocating for students with special needs is preferred
For teachers - Current State of Tennessee Teaching or Administrator License is required; or
For School Psychologists – licensure as a School Services Personnel at the Professional level in the state of Tennessee or Tennessee licensure as a psychological examiner or Psychological Assistant is required; or For School Social Workers - licensure as a School Services Personnel through Tennessee Department of Education or certification as a Certified School Social Work Specialist through the National Association of Social Workers required
